Leigh Ann is a Certified Public Accountant with a Master’s in Business Administration. As a 12 year veteran of the rehabilitation industry, she possesses an expertise in Medicare reimbursement and invaluable knowledge in navigating through reimbursement issues. Prior to being named VP of Finance in 2002, Leigh Ann served as Paragon’s Controller for eight years. During this time she directed the creation of and managed the implementation of Paragon’s accounting and billing systems to achieve efficiency throughout the organization.

Mindy, a resident of Northern Indiana, has over 20 years experience in the healthcare arena with 16 years of management experience in rehab settings, including outpatient clinics, hospitals and skilled nursing facilities. Mindy’s expertise in accrediting and regulatory agencies includes CMS, CARF and Joint Commission. Her expertise provides the background for coding, billing, documentation, reimbursement and clinical programming. Mindy calls upon her extensive experience with Medicare and the fiscal intermediary process of medical reviews in skilled nursing facilities to reduce the number of denied claims and achieving successfully appealed payments. Her focus is to ensure quality programming and 100% compliant documentation. Mindy graduated with Departmental Honors from Indiana University with a degree in Occupational Therapy. She is an active member of AOTA.

Denise is a graduate of the University of Evansville as a Physical Therapist and has had 20 years experience as a clinician and 18 years in Management. She was the owner of her own private practice specializing in Orthopedics, Geriatrics and Ergonomic Consulting. Denise is a Wound Care Certified clinician by the National Alliance for Wound Care. She does wound consulting and teaches appropriate wound care to therapists and nurses. Denise researches and develops clinical education programs in all areas of treatment for Paragon. She specializes in the accreditation of those programs and training our Paragon clinicians. This insures Paragon therapists will deliver state of the art treatments to our patients. Denise is an active member of the APTA, Geriatrics and CEMS Sections.

Marilyn Winkel, OTR

Marilyn has been with Paragon Rehabilitation as a Program Director since April of 2005 and was promoted to Clinical Support Specialist in December of 2009. She received a Bachelor of Arts degree from Purdue University in 1993, and Master of Science in Occupational Therapy from the University of Indianapolis in 1995.  Prior to joining Paragon, Marilyn’s experience includes clinical and management roles in acute care, outpatient and industrial rehab, private practice, and long term care. She is skilled in clinical documentation, coding and billing, and the Medicare medical review process. Marilyn enjoys sharing her knowledge and experience with program directors and therapists in order to achieve quality clinical documentation.

Bill has 25 plus years in the healthcare industry including positions as a medical billing/management consultant and software developer, Executive Director of a Rural Health Clinic, and immediately prior to joining Paragon Rehabilitation, as Marketing Director for the nation’s largest independently owned contract rehab company. Bill enjoys working with our Paragon customers and partners when developing aggressive business sustaining and growth strategies.

Madhu obtained his BPT degree, physical therapy from MGR University and began his career as a therapist at Heritage Therapy Services. Afterwards, he accepted a position with the nation’s largest contract rehab company where he served as Director of Operations. In 2006, Madhu accepted his current position with Paragon. Among Madhu’s other operational responsibilities is his role as our 'CarePoint' software coordinator and IT special analyst. Madhu is an active member of APTA and the Indian Association of Physiotherapists.

A graduate of Indiana University with his degree as an Occupational Therapist, Ron has over 16 years experience in the healthcare field and has managed some of the largest therapy providers in the USA. Prior to his employment with Paragon Rehabilitation, Ron worked as a Director of Operations for the nation's largest rehab company where he directed the rehab activities at hospitals, long term care facilities and independent physical therapy facilities.

Andy started with Paragon full-time October of 2006 and was promoted to Area Manager in April of 2009. He graduated from the University of Indianapolis with a BS in Athletic Training, and Washington University School of Medicine with a MS Physical Therapy in 2002. Andy is currently in progress of a DPT from Washington University. He has six years of clinical and management experience in outpatient clinics and rehabilitation facilities. Andy has a focus on program development, marketing and promoting rehab professions with community outreach programs. He is a member of the American Physical Therapy Association.

Cheryl, a resident of Wilmer, Alabama, has over 20 years experience in long-term care with 17 years experience in skilled nursing facility management. She graduated from University of Alabama Birmingham in the PTA program. Cheryl has worked as a Program Director since May of 2005 at Plaza Community Living Center on the Mississippi gulf coast. Her focus is exceeding customer’s expectations. She was promoted to Area Manager in August of 2009.

Mary began with Paragon as Area Manager in May of 2009. A graduate of Wallace State Community College in Hanceville, AL, she has an A.A.S. in Occupational Therapy. With 10 years clinical and  management experience, Mary is experienced in denials management and successfully assisted a facility in being removed from a focused Medicare review. Prior to joining Paragon, Mary served on an Employee Action Committee which developed company policies and procedures while improving employee satisfaction. 
 
Paula Rabbeth, OTR/L, CTL

Paula was promoted to Area Manager in May of 2009.  In 2000, she graduated from Spalding University in Louisville with a BS in Occupational Therapy, and from Bellarmine University with a BS in Economics. She has experience in outpatient, pediatrics and geriatrics. Paula has seven years clinical and management experience in rehabilitation facilities including the Program Director position at Glen Ridge in Louisville, KY. Paula has focus in lymphedema management, NDT, positioning and documentation and has even developed courses on documentation with approval from the Kentucky Board of Occupational Therapy to award CEU's.


Sara Durham, MOT/OTR

Sara joined Paragon as a Program Director in 2004 and was promoted to an Area Manager in 2010.She has a Bachelors of Psychology from St. Joseph’s College in 1993 and Masters of Occupational Therapy from Nova Southeastern University in 2003. Prior to joining Paragon, she served on the CVA team at a Joint Commission disease-certified stroke rehab hospital. Sara’s experience is an asset to Paragon. Sara is skilled in clinical documentation and the Medicare medical review process. At Paragon, she participated on the “Paragon Service Standards” and the Program Orientation manual committees. Sara is a member of the American Occupational Therapy Association and Indiana Occupation Therapy Association.
